General Ambulatory Care Clinic Referrals

General Ambulatory Care Clinic referrals are triaged by a clinician. It is essential that all referrals include information that will allow the triage process to occur in a timely manner.

All referrals should include the following information:

• Patient’s full name
• Patient’s full address
• Patient’s telephone contact number’s – (home, mobile)
• Patient’s date of birth
• Patient’s Medicare number
• Interpreter required? If yes, language spoken
• Name of specialist to whom the patient is being referred
• Reason for referral
• Medical History
• Relevant diagnostic results and current medications

Once the referral has been triaged the patient will be notified by mail or phone of their upcoming appointment.
