These patients often require comprehensive assessment, care planning and review to prevent unplanned hospital admissions. People who are aged 16 years or older Able to self-manage or have carer Diagnosed chronic and complex health condition/s Within St Vincent’s Hospital Sydney Catchment Area Patients outside St Vincent’s Hospital catchment are not eligible.
