Tips for facilitator

  • Remind team that this is a short 10 min debrief, so not the time for detailed analysis. A more detailed unpacking of the incident and corrective actions will be included in the incident review/Riskman process 
  • Be mindful that people’s emotions and experiences might be different in this moment and the aim of the hot debrief is to be supportive and show care for one another, this is a ‘no blame space’
  • Team is asked to be respectful of each other and take turns to speak 
  • If you have any concerns regarding individuals or the team after the debrief, escalate through appropriate channels – line manager, Safety Wellbeing & Recovery, or P&C team

  • This is a safe opportunity to learn and support each other when things go well and when they don’t. It’s a no judgement, no blame space

  • Everyone reacts differently after an incident. You may think a lot about this in coming days, feel a range of emotions and even physical symptoms. You may even rethink your career. All of these are normal reactions and should pass with time. Reach out for more support if needed.
